When life presents us with challenges, like the loss of someone we love, interpersonal difficulties, marital or family issues, or job stress, we may feel like we are alone.  During challenging times, Transitions can help!  For 70 years we have provided behavioral health and rehabilitation services to help individuals and families during difficult times.

We provide a comprehensive array of services for people of all ages and all abilities. Our services are accessible and affordable - we accept private insurance and Illinois Medicaid.   For those with neither of these sources of payment, fees are established on the basis of family size and income.  

Transitions provides the following services ... 

*  Counseling/Therapy Services

*  Substance Use Treatment, Including Medication Assisted Treatment

*  Psychiatric Services

*  Case Management & Care Coordination Services

*  Crisis Intervention Services, including Mobile Crisis Services

*   Services for Veterans & Their Families

*   Recovery Services, including peer support for those who have a serious mental illness

*  Vocational Services, including job placement & support & job coaching

*   On-site Primary Care Services, in partnership with SIU School of Medicine

*   Parents as Teachers Program for pregnant mothers and for families with a young child aged birth to five.

*   Special Education Services for youth who have autism spectrum and/or other communication disorders

*   Community Day Services for adults who have an Intellectual/Developmental Disability (IDD) 

*  Residential Services (group homes and supported apartments) for adults who have a mental illness or IDD
